ICC Referral

The ICC information is obtained from the current events HIA assessments and the player's profile and history. However, if there is no HIA3, the ICC cannot be conducted.

To begin the checklist, you must first confirm that the player is fit to return to play. If you believe that the player is not yet ready to return, the ICC referral cannot be completed and the player's rehabilitation should continue.

Please answer all questions that require a response and proceed with the assessment. You can choose your desired ICC from the dropdown list, which can be filtered by country and language spoken.

After selecting the ICC, provide the proposed date of the next game and any additional comments you want to add.
